Microsoft Forms (or MS Forms) is an online tool that your instructor may use to check in, collect feedback, collect responses in real time, or conduct peer assessment.

How to Access MS Forms

If your instructor wants you to complete an MS Form, they will provide you with a direct link to the form. If you’re being asked to complete an MS Form in a classroom setting, they may provide a QR code for you to scan with your mobile device.

Note: You may need to login with your O365 credentials to access the form.

Completing a Form

Follow the link provided by your instructor and simply fill out the form.

Be sure to click Submit when you are done to ensure your response is collected.
